Penile lengthening surgery is performed under general anesthetics. Summarily, a patient’s internal part of his penis is released by the division of the ligament holding it back. A muscle technique is implemented to ensure a long-term solution is performed during this surgery. In some cases, liposuction can be done in areas above the penis if the area is full, also increasing the length.

The purpose of the penile lengthening surgical procedure is to increase the external length of the Penis. There are other techniques such as the peno-scrotal cutaneous plasty, glans filling, and supra-pubic Vibro-lipo-sculpture that can be also be used to increase the length of the penis. It is also possible that during penile lengthening, it is possible to perform penile girth enhancement using the lipo-peno-sculpture method.
Here are the details of the procedure:
The release of the suspended penile ligament and inverted anti-retraction sutures.
This step involves the release of the suspensory penile ligament. The said ligament is located between the internal penis and the pubic bone. Its function is but not limited to joining the internal parts of the penis and the pubic bone resulting in the sliding forward motion of the internal part of the penis.
The length achieved during this surgery largely depends on the consistency of the suspensory ligament. This means that a more developed penile suspensory ligament gives a better result in regards to penile lengthening. In fact, it is proven that the strength of the suspensory ligament and penile length are directly proportional.
It is however impossible to know the strength of the suspensory penile ligament before surgery. Why? This is because this ligament is situated deep in the groin area, which is not easily revealed via an ultrasound.
The strength of the ligament and length achieved just like other organs in the body varies from person to person, making it difficult to have a correct prediction of what can be achieved during and after surgery, however preforming the penile lengthening procedure alone can guarantee a boosted penis length between 2 to 4 cm when the penis is flaccid.
Modern medicine has discovered a predictive means of measuring penile length achievable. This is done by measuring the visual part of the ligament via the use of ultrasound, and this can be compared with statistical data available within our reach.
During the procedure, the aspect that involves the visible suspensory penile ligament is easy and does not involve any extensive technicalities. The aspect to be paid more attention is within the deep parts of the ligament which is close to the dorsal neurovascular structures in the penis.
Due to the sensitivity of the revealing inner part of the suspensory penile ligament and its proximity to the dorsal neurovascular structures, we use the latest innovations such as a computer-controlled diode laser; known to provide precise cuts, and limiting pain and blood loss during the procedure by cauterizing the cut at the time. This also reduces the healing time.
A small horizontal incision is made in the pubic area at the base of the penis, an area without skin flaps. The skin is then stretched, no incision is made on the penis, and the release of the suspensory penile ligament can then be conducted.
In most cases, the length is usually partial due to a ‘scar retraction’ of the ligament most patients frequently undergo. This retention causes a sealing days after the surgery, canceling out the results of the procedure, whose intensity varies from patient to patient.
To avoid such complications like this to occur, extensors are used in the penis, highly recommended by the medical community. These extensors are used to mechanically control and help with the progressive traction. Though sold alone to help in the extension of penises without surgery, it has been confirmed to be ineffective alone without surgery. But their effectiveness cannot be overemphasized in post-surgery situations; in helping in the prevention of post-surgery scar-retraction. It should be noted that this should not be used in the first few days after the surgery due to the presence of scars from the surgery, limiting their efficacy. They can, however, be used after 30 days.
This technique has been modified by us to provide the best most permanent results. This surgery has a duration of about one hour and is done with the patient under general anesthetic depending on if other methods are being applied during the operation.
Supra-pubic cutaneous laser-plasty
This is the next stage after the laser release of the suspensory ligament in the penis if this is done as a second procedure. This procedure is done to contextually lengthen the skin that covers the penis, which in most cases is short due to the release of the ligament.
How is this done? This procedure is carried out via a process called the V-Y plasty. This simply means in layman’s terms as an incision that is made in the shape of an inverted V, and after the surgery is done, it ends up being an inverted Y.
A diode laser is used to improve precision of the incision, allows for less bleeding and reduces stress upon the tissue around the area, resulting in a faster healing time of the surgical wounds.
To suture the cuts, an aesthetic suture with resolvable stitches is used, this means the sutures will fall off on their own in 15 to 20 days after the surgery.
In some patients, there are evident and an abundant amount of supra-pubic adipose panniculus, this can exceed the dermal insertion of the penis, leading to the penis visually looking shorter than it is. This condition is mostly restricted to overweight patients. However, normal weight or even under-weight patients can also suffer from this caused by personal characteristics of adipose accumulation zones.
In a case were penile lengthening involves supra-pubic adipose panniculus, this technique helps in the reduction of fat, especially pubic fat. Using cannulas that are thin and fitted with a vibrating mechanism that helps in fat dissolution and then aspiration of excess fat.
This vibratory procedure is important because the male adipose tissue has greater consistency than that of the female and can be easily removed using normal lipo-sculpture to flatten the supra-pubic area. This in turn increases the length of the penis. This procedure can be carried out under general or local anesthetics and largely depends on if the procedure is done alone or with other procedures.
This procedure is done with two small incisions in the skin beside the area that treatment is needed and a thin cannula is inserted. The cannula then vibrates, dissolving the fat and suctioning it out. The procedure takes around 40 minutes, depending on the amount of fat to be removed. Same day discharge applies to this procedure.
